Counselling and Clinical Supervision Services
As part of providing a quality service to you, I will need to collect and record personal information from you that is relevant to your current situation. This information will be a necessary part of the assessment, planning and intervention.
You may access the material recorded in your file upon request, subject to the exceptions in National Privacy Principal 6.
Confidentiality. All personal information gathered during the provision of services will remain confidential and secure except when:
1. It is subpoenaed by a court, or
2. Failure to disclose the information will place you or another person at risk; or
3. Your prior approval has been obtained to:
a) Provide a written report to another professional or agency. Eg. A GP or a lawyer; or
b) Discuss the material with another person. Eg. A parent or employer.
Strict Cancellation Policy
If you need to cancel or reschedule your appointment, please provide at least 48 hours notice. Full payment of the appointment will apply for appointments that are cancelled or reschedule less than 24 hours before the schedule appointment time and date or if you do not attend the scheduled appointment.
The payment will be processed automatically using any available payment methods provided by you and any outstanding fees must be paid prior to making another appointment.
